Shortly after bringing this home a today I discovered a defect in the frame just below the hand. This seems to be preventing the cylinder from rotating clockwise far enough to engage the cylinder stop. It will only lock up when rotated counterclockwise after closing the cylinder. This is a brand new factory fired only revolver. I haven't even dry fired it.

The star also has some unusual wear

It is a really nice revolver but I'm pretty bummed I've got to send her in before I can shoot her. I missed the problem at the gun shop because I've never seen it before. Has anyone else ever had this same issue?

Could you be a little more specific describing your defect? I don't see much difference between your gun and my stock 686. Your hand seems a little longer, but maybe its the light or camera angle. Good luck with your new 686. I hope you get it sorted out without a return to SW.

So I'm not exactly sure what the issue is but I don't think the burr on the frame under the hand is supposed to be there, and I think the hand is supposed to sit slightly sub-flush when the hammer is down. The hand sits slightly proud in this case and will catch the star when the cylinder is closed.

For reference, here is a picture of my 629 that has 3000+ rounds through it. Notice the clean square edge, and the sub-flush hand.

These 7 shooters seem to be problematic.

Too bad you pay so much and S&W doesn't check to see if it works before they send it out.

Bring it back to the gun shop! Brand new and Never been fired by you then he needs to make it right, exchange it for another.
This is a good reason to by local, you can return it to the shop and they get to deal with S&W.
Definitely a burr from the factory and should have been caught before leaving the plant.

My 686 does not have that burr under hand. I'd take it back to the shop where I bought it. Maybe they can exchange it for another one.
