686 won't shoot good groups

I finally had a chance to retest the 686 after thoroughly cleaning the bore. Using 158 gr. Sierra JSP at 25 yards I would not call it a group. Note the two circled in red as they are starting to tumble. Do I find a good smith or send it back to the factory?

That's an awful lot of vertical stringing. You sure you're consistent from one shot to the next?

I posted a picture of this one earlier. My brother competed with it.
This is how it came to me but it was much dirtier. This was after its initial cleaning.
I was wearing Hogue Mono grips, has a Millet rear sight and a smooth
target trigger.
I think the trigger has a micro-ounce pull too. :D

Its the most accurate gun I own. ;)

I was pretty sure I was more capable than that and only at 25 yards. Now I am shooting 3/4" groups at 50 yards with it. I am really surprised at how picky this 686 is.
