Can anyone assist me with this insurance claim

My Wife is receiving home hospice and so far everything has been paid, then I get this in the mail yesterday. I am contacting them first thing in the morning to inquire if we owe anything, my Wife is distraught over this and just trying to see if anyone could clear it up for me. Thanks

It looks like somebody messed up when the claim was filed and it was just denied as a standard procedure. Not familiar with Humana but everything should be covered when under Hospice care.

It is the responsibility of the medical care provider (hospice) to bill your carrier and see to it that the claim forms are filled out correctly. Contact them first. If should get this cleared up especially if they have been paying the same provider for the same care before.
 
I believe that you all are correct, this is something that the H.H. has to sort out, I will not worry about it. thanks for helping!
 
Humanna is a "Managed Care" company. If you are under 65 this can be a group contract through employment, etc., if so contact your employer who should have a Insurance Rep on staff or available to assist all beneficiaries with claim problems. Humanna also is a Medicare Manager care company, and if you have it direct via an over 65 plan, check your policy as they should also provide you with a contact person/advocate to assist with any claims. In order to file claims, a Provider must be "Under Contract" with Humanna. Don't wait, contact them right away to resolve as soon as possible.
