41s get dirty in the area of the breech block (a separate insert that is pinned into the "slide") and extractor. It is entirely possible your extractor is a bit crudded up and is not able to function normally.
If you have not done so lately, it might be a good time to disassemble your slide and thoroughly clean everything. I think you will find it is worth the trouble to remove the breech block and extractor so you can do a good job of cleaning. You really can't do it otherwise.
The breech block will sometimes peen itself into the slide very tightly and be difficult to remove. Put some Kroil on it and work with it (wiggle it around as best you can) and it will eventually come free. When you reassemble, you may want to stone or lightly file the peened edges of the block so it can be returned to its normal position without undue force. Don't get carried away with this.
I think cleaning will solve your problem.
