My guess is that at a minimum, he sustained bruising to the lungs (pleural contusions). Lungs don't like being contused and beat upon, and they start leaking fluids, especially blood, (pleural edema). Despite his protests to the contrary, I think the round, or perhaps pieces of the bullet jacket did penetrate the skin of his chest wall. He probably has the constitution and survivability of a cockroach, so this won't kill him, but he doesn't feel quite as spry as he is letting on.
No, I'm not a doctor, nor did I stay in a Holiday Inn last night. I've just seen quite a bit of trauma in almost 20 years of working as an EMT and an ER Nurse.
