It is not a ballistic failure, it is a failure as to proper targeting. It has been discussed heavily on a couple of more hardcore forums. The proper target on a frontal headshot is the triangle formed by the outside edges of the eyes and the upper portion of the bottom of the nose.

While I don't care to rely on my thick skull for defense, that is in fact it's design, and anyone who relies on a handgun bullet to penetrate that is a fool. In addition, the correct portion of the brain to hit is below the forehead.
