I was wondering if it would be possible for a gunsmith to install an adjustable rear sight on my 638, the stock sights are good but not my favorite

OK - you are crazy... adding adjustible sights would be cost prohibitive even if it would work. A bodyguard is not a target gun. If you want adjustible sights on a J frame, look for one of the 3" model 60's.
