1. "Been there - Done that".
2. I wish people would stop saying "axe" when they mean "ask".
3. I cringe when I hear reasonably intelligent people pronouncing the word "nuclear" as "nuke-u-ler".
4. People that end sentences with a preposition - for example, "I was lookin' for you, where you been AT?" instead of "where have you been?".
5. Educated people who can't seem to pronounce the state of Massachusetts properly; I often hear it pronounced Massachusus or even Massatusus.
