I don't think so.
The gun was a 6906 (The gun in the photo is a 4006 but it's functionally the same gun) I was doing a TRB and I engaged the safety. From what I've heard it's a fairly common issue with 3rd Gen S&Ws.
Bought this a week later and haven't had that issue since.
