I have one too. It came with the tool kit for my adjustable sight 659.

I have/had some of both styles (little hole/big hole). IIRC, they came with LSI era guns.

I have a couple of the two blade and a 4 blade somewhere. I keep a two blade on my key ring and use it quite often. Real handy tool. They must have come in revolver boxes as i have never owned a S&W auto.
