UPDATE. There is now a video out there, with a employee taking a customers cart and card away as he refused to wear a mask, "You are no longer welcome here, please leave"

I logged into my COSTCO account and found their "Feedback" link on the bottom page banner, and told them I supported their protective measures and the actions taken by Tison.
