M&P Pro fails to fire after mag change

I'm a new USPSA shooter using an M&P Pro 5" that I modified with an Apex trigger spring kit. In the past two shoots I've had a situation where after a mag change with a round in the chamber the gun failed to fire and I had to work the action. It happened to me three times last weekend and twice last night. Any ideas on what might be causing this? I'm wondering if I'm not seating the mag but the gun works fine after racking the slide. I modified the trigger last year and never had this problem until recently.

If you're in or near Oklahoma, we have an M&P guru that keeps a lot of competition guns running. If not, you may need to find one.
This is just a SWAG, but my guess is you have problems in the trigger/sear interface. Your modification was possibly barely working, and some wear plus extra pressure on the bottom of the slide (you're shooting limited and reloading with 17?) may be just enough to give you a dead trigger. I am not a gunsmith, but your solution may be as simple as an adjustment or a spring replacement? Might call the APEX folks for advice.
