Why of why would someone use/ carry/ deploy/ operate a handgun that is stated as defensive/ mission critical with an APEX trigger in it? OEM setup on Defensive Guns, ONLY. Apex, Dynamic, and the like are made for Competition use. there are so many reasons to never do those things...
Apex and Galloway both offer triggers that just replace the factory trigger without changing any other part (for people, like me, that just don't like the hinged trigger).

O.P. doesn't say if it's just a replacement Apex trigger, or an Apex trigger kit (that changes several other fire control components as well).
 
Unfortunate situation while at work, I was inserting a fresh mag into my pistol when it fired and struck me in the leg. I inherited this gun from my grandfather who had a gunsmith replace the trigger to a APEX trigger. We’ve had several visits at the range no problems.
This shouldn’t have happened. Our office was full of people standing around and there really wasn’t a safe direction to point it other than towards myself.
What the ****! Would cause this to happen?
I was aware that there was a round in the chamber, but have inserted mags hundreds of times this way with no bad results like this.

Maybe the OP needs to switch to revolvers so as not to tax his safety knowledge. No need to mess with the cartridges in any way unless he's at the range for a practice session.

And for pete's sake, what possible reason is there for handling ANY firearm in an office full of folks?

I doubt this event actually happened.
 
One day at the range I was handed a Glock with a trigger job. I barely touched the trigger and it went off. Hit target nearly bullseye.

But I did touch the trigger.
 
I keep picturing McGee on NCIS getting up and pulling his pistol out of his desk drawer and inserting a mag, on his way to track down a bad guy. Pure Hollywood.
