I had exactly the same problem. I believe there are two issues here. One, wimpy loads that don't have enough oomph to fully cycle the slide. I could see the weak ejection and know that there was an empty chamber on the next shot. Two, lubrication. I tried Federal bulk, copper and lead, Winchester Wildcats, Remington Thunderbolts, with the Federal being the worst. After about 2K rounds it has cleared up nicely, with a miss feed every 100 rounds or so. The biggest improvement was lubing the gun. I was using Gun Slick grease and had problems galore. Went to Rem oil and things got better. Went to synthetic 30W auto oil liberally applied to the suggested points and it really worked well. I think it just takes breaking in. Another suggested alternative fix is to have a good gunsmith make a lighter recoil spring for it, though this would be a last ditch fix. I think the 2k rounds has actually broken in the spring so it is a tad lighter. My gun now runs just about everything I put in it, but it certainly tried my patience at first. It's a great .22, the most fun one I've ever shot, so hang in there. I talked to S&W a couple times and they were willing to take it in for repair. I never got to that point. PS. I just ran a couple hundred rounds of Winchester EXPERT HV through mine with no problems at all. These are at wally world for 17 bucks for 500. They are lead hollow points.
