You need to specify which size you are referring to:

Rear sight length or rear sight blade height, or both?

The early rear sight tang length is different with 2 1/2" barrels from other K and L frame barrel lengths; it extends all the way to the front of the frame and joins the back end of the front sight base.

I see that there are two types listed. One is for one that is drilled and tapped. What does this mean ? I currently have the gun in the 10 day wait period, so I can't look at it right now .Thanks again.
 
686-4 and up have 3 holes tapped in the top of frame by the factory for mounting rails for optics . The sight assembly covers them and held in place with one screw the first hole towards barrel. Info on S&W 686 can be found on Wikipedia with list of dash numbers and changes.
