I like my steak cooked in an iron skillet well done with the fat left on it so that it stays nice and juicy.

I guess I'd say somewhere between rare and medium.

The scale of your poll is incorrect.
Rare = Cool red center
Medium Rare = Warm red center
Medium = Hot pink center
Medium Well = No pink in center
Well = Why did I order an expensive steak when a hamburger cooked like this would be as good.

This is MY scale and it IS correct for me.
When I place my steak order and give my little speach there is seldom a problem understanding how I want it done. 
I once was a well done to burn it afficionado
now in my 70's and fewer teeth I prefer medium

If I didn't know how bad it is for me, I would still do my old favorite.
Fork - knife - salt shaker, steak right out of the fridge, and on the plate. Perfection!

if I order rare I usually get medium rare which is ok...once in awhile I get rare

I have noticed a great deal of regional variation in what the terms mean. On a couple of recent vacations ordering a steak in Waco medium rare resulted in a quite different product than the same in Spokane.

LOL as much as it's hard to get a rare steak its just as hard to get a good medium well. I usually cook my own on the grill but when I do order out its "medium well, NOT BURNT". Once upon a time I went off on Charlie Brown's in NJ after one too many ruined steaks... something along the lines of not being able to cook anything other than burnt to a crisp or dripping blood.

At home, I'll usually put my steak in the freezer for 30-60 minutes just before I cook it, so it will be rare enough when served.
