How do you guys answer this?

I find it very hard to believe ANYONE could be as stupid as to do that to you. "NORMALY" a consent form has to be signed by the person getting the shot PRIOR to it being given.

If you're referring to Chrissy giving me that surprise flu shot, I don't think my doctor has ever asked for a consent form for anything either....

OTOH, I've been asked for consent forms for surgical procedures, and my dentist asks for one when contemplating an extraction.

While the Chrissy thing is a little off-the-wall, and I don't trust/like the doctor at all, my doctor is his friend, so I didn't even think about it. IMHO, if you turn up by way of an appointment, you've really given consent anyway. The forms are just CYA.

Last time I was admitted via the ER, I don't recall any forms like that, but that was in 1999. Seems like when I was taking mom to the ER with some regularity (after 2007, but I can't give a date), one of us did seem to sign something. Her if she was conscious and apparently in possession of her faculties, and me, otherwise. (We can argue about her faculties if she let me sign :D.) I checked myself into one of the hospitals for a necessary bit of surgery in 2005 - lots of stuff to sign that time :D.... Elective, but necessary. (Meaning that waiting another couple of weeks wouldn't have hurt much, although inadvisable.)

Overall, though, our problem really is that when some yutz in North Dakota steps on a bug, politicians in DC will decide to make that a key part of their effort to ban shoes for folks living in all states. There is absolutely nothing that can be done about the occasional nut case, except to minimize the free fire zones.

The anti-freedom folks are scared to death of us NOT being sheep - it's all about CONTROL. They'd be on our cases to ban shovels and rakes if they thought we'd even think about not obeying them....

I went to get a eye exam last month and the optometrist asked if i had noticed any "flourescent dots " on my mailbox.
 
I just got back from the clinic for some lab test & when I checked in I was handed a short sheet of paper with a few questions on it & a place to sign & date it. I thought this was going to include "THE" question on gun ownership. Nope. Just an update because the last time I was there was just before Christmas. I get to see my Doc on Wednesday to get the results & some counseling but since I already filled in the form all I need to do is check in. Doc knows I have guns & never asked me since I volunteered the info last year about being excited because on my way home I was going top pick up my new M&P40C. We had a short discussion on carrying guns & he seemed interested & he mentioned that was something he was thinking about. Of course he knows I'm retired Military & was working in armed security until recently.
 
I find it easy to just say "no" and move on to the next question.
I don't feel that my rights have been threatened any more than when they ask me if I have had Tuberculosis.
 
I have been asked this and responded "No, but I'm curious... why would you ask about guns in the context of this office visit?"

My take is that there is no reason to fan the flames, especially if the Dr. is required to ask this question by an employer's policy.

My answer is going to be no. If challenged on it later just say the Dr. must of misunderstood my answer. If it's a written question, it's still going to be no. Answer like the libs do. My eyes are bad, my hearing, I was tired, my feet hurt I was in pain, it's not my fault, why does in matter now anyway.
 
In the Navy all hands know that a gun fires a projectile 1" or larger. So no, I do not have any guns at the house, sadly.
 
I had to pick myself up off the floor from this one. My Uro and GP shoot with me almost every week. We are all .45 ACP nuts.
