I know the gun is under warranty, but if you have a gunsmith nearby, I would have him take a look at it.
The mag follower is what pushes up the slide lock after the last round is fired. If you put in an empty mag, the slide should lock back every time, otherwise, it should never lock back with ammo in the mag.
From a sand bag, you shouldn’t be having trigger control issues (some folks pull their shots and I had a new 9mm Shield that shot left 2” for everyone that shot it from a rest). Have someone else shoot it and see if they have the same results you are getting. I check all my new guns groups at 10 yards, but that’s me. You shouldn’t have to move your rear sight that much. JMO My 9mm EZ sights were right on horizontally but shot low 1” if you used the three dots (factory miss drilled front sight dot). If you lined up the top of the front and rear sight it was POA=POI. I painted the front sight yellow and painted back sight dots with Testor's flat black model paint.
If it’s something simple, the gunsmith should be able to find it, then he can fix it or tell you to send it back to S&W. JMO
