The pictures are out of order. 1 round fired and that's the one with the brass exploded. Was at the top of the mag when the mag was removed to show clear to make the range safe. (pic 3) Then I took it out and moved it around trying to reconstruct (pic 1). I wanted to send the mag with the pistol to the factory but was instructed to _not_ send the mag. I still have the mag on my bench in case it comes to that.Well, the above would definitely be associated with a magazine malfunction... There are two cases that appear to have been affected: with one stuck on the "rectangular rail" and one pointing down into the magazine. How a projectile not fully seated in the chamber went down the barrel is interesting as is the round with the dent pointing down in the magazine. I presume it is unfired?
As far as the OP's post is concerned I suspect a little green man under the bridge might have an explanation...? Just an IMHO.

Factory SW mag with less than 5K on the mag spring. Factory mag springs. In general, my practice is to run factory parts for everything because I don't know enough to diagnose anything but the most blatant mechanical issues. One bad ProMag experience years ago taught me the difference between price and value when it comes to parts.
Yep.. high primer is my first thought. Even if it was new factory blazer and not a reload, these three came out of the same fresh box from Blazer just a couple of weeks ago. No guarantees they couldn’t mess up the other side of the round.. I was literally at my kitchen bar loading up magazines for the next day’s range trip…when something didn’t feel right…Got that. I see a Blazer headstamp on the damaged round. Factory fresh or reloaded? Just wondering about the primer and if it was sitting high.

Good on you!1981 a person discharged their weapon unsafely and put a .357 into my right foot. 1992 a person field striping their Glock 19 dropped the mag and then depressed the trigger as they attempted to remove the slide sending a round in front of my face and a couple other people. 1996 had a person bragging about their new Beretta decide to pull it out and wave it in my face. That was enough of that crap, he got knocked out stone cold on the spot. I've absolutely zero tolerance for any and all stupid gun handling now. Since that last incident I've "greatly" limited where I shoot, and who I'll shoot around . . . . . .
I think anytime non stock parts are replaced it is a safety issue. I never replace stock parts on a carry gun other than sights.Unfortunate situation while at work, I was inserting a fresh mag into my pistol when it fired and struck me in the leg. I inherited this gun from my grandfather who had a gunsmith replace the trigger to a APEX trigger. We’ve had several visits at the range no problems.
This shouldn’t have happened. Our office was full of people standing around and there really wasn’t a safe direction to point it other than towards myself.

Absolutely this. No way would I carry or use that firearm until it had been thoroughly stripped down and diagnosed.Just speculating here, but I suspect when the apex trigger was installed the striker/firing pin safety was removed in an effort to smooth the trigger pull. A very foolish thing to do in my opinion but it has been known that people sometimes do this. I would have the pistol disassembled by a qualified armor, inspected, and replace any missing parts.
If you are standing, why not spread your legs a little and point the muzzle toward the floor between your feet?The type of work I do, it’s dangerous.. I was getting ready to leave out in the field and the safest place to point was towards my led so it wasn’t anybody else’s.
Police report stated malfunction, as I had six witnesses and standing around me.
