M&P 9 fired while inserting magazine

1981 a person discharged their weapon unsafely and put a .357 into my right foot. 1992 a person field striping their Glock 19 dropped the mag and then depressed the trigger as they attempted to remove the slide sending a round in front of my face and a couple other people. 1996 had a person bragging about their new Beretta decide to pull it out and wave it in my face. That was enough of that crap, he got knocked out stone cold on the spot. I've absolutely zero tolerance for any and all stupid gun handling now. Since that last incident I've "greatly" limited where I shoot, and who I'll shoot around . . . . . .
 
I had a colleague who had a negligent discharge while on duty. Fortunately, it was just him and the witnesses were inside a building but saw the whole thing. The front office handled it. A couple of years later, he and I were on a trip and sharing a drink when I got up the courage to ask him. "How did this happen? Guns only go off when you pull the trigger." He looked me straight in the eye and said "well." That is all I have to say about all this.
 
I'll add my story.
MP 2.0 Compact 9mm - hinged trigger version, no after market internal parts (milled for dot and sights replaced). Slide lock replaced with a new one from current production with the extended slide lock lever (lefty friendly). About 7K rounds on this RSA and 8K on the striker assembly. 23K total on the pistol.

IDPA match, shot to slide lock and inserted a fresh mag. Slide auto forwarded (not uncommon for this pistol) and the forward tip of the center rectangular rail in the middle of the slide hit the primer. My finger was not on the trigger. RSO confirmed finger was off the trigger and on the frame. Round discharged before fully entering the chamber. Bullet did go through the barrel and downrange. Round impacted in the side berm of the range just under the target.

SW has had the pistol since Nov 17 for inspection/repair. I'm getting very frustrated with them. If it is going to take this much time, just replace the slide under warranty and then spend however long they want on inspections. We'll see if I can successfully add images to this post. Anyone have any ideas as to why this happened?

Well, the above would definitely be associated with a magazine malfunction... There are two cases that appear to have been affected: with one stuck on the "rectangular rail" and one pointing down into the magazine. How a projectile not fully seated in the chamber went down the barrel is interesting as is the round with the dent pointing down in the magazine. I presume it is unfired?

As far as the OP's post is concerned I suspect a little green man under the bridge might have an explanation...? Just an IMHO.

Well, the above would definitely be associated with a magazine malfunction... There are two cases that appear to have been affected: with one stuck on the "rectangular rail" and one pointing down into the magazine. The pictures are out of order. 1 round fired and that's the one with the brass exploded. Was at the top of the mag when the mag was removed to show clear to make the range safe. (pic 3) Then I took it out and moved it around trying to reconstruct (pic 1). I wanted to send the mag with the pistol to the factory but was instructed to _not_ send the mag. I still have the mag on my bench in case it comes to that.
Factory SW mag with less than 5K on the mag spring. Factory mag springs. In general, my practice is to run factory parts for everything because I don't know enough to diagnose anything but the most blatant mechanical issues. One bad ProMag experience years ago taught me the difference between price and value when it comes to parts.
 
You broke so many safety and procedural rules/guidlines, I don’t know where to begin, so I won’t.
I suggest you take a class in gun safety by someone that knows what they are doing!
If your company allows you to load or unload a pistol in an unsafe area (people present), they should have a bullet trap for you to use.
If it was really a malfunction of the pistol, how often do you maintain your weapon? They do get lint and dirt in the works.
You were very fortunate no one was shot/killed and you survived the wound.
Chock this up to a learning experience!

Got that. I see a Blazer headstamp on the damaged round. Factory fresh or reloaded? Just wondering about the primer and if it was sitting high.

Yep.. high primer is my first thought. Even if it was new factory blazer and not a reload, these three came out of the same fresh box from Blazer just a couple of weeks ago. No guarantees they couldn’t mess up the other side of the round.. I was literally at my kitchen bar loading up magazines for the next day’s range trip…when something didn’t feel right…

On the chance that he is legit, I want to say,

Or my OTF knife fired in my pocket and I never touched the button .

Only reason I carry a folder is NC knife laws so for the last 15 years my carry knife has been a ZT 0350 . It is heavier than many folders but after carrying a commander as my EDC most of 35 years the weight is still lite at about 6oz . Specially after dad gave me a case 6265 back around 1968 I think with a raw hide loop that made for a good black jack too . Then I had , still have a schrade152ot old timer . Cheap but old ole knife that I bought went I was around 18 . Pulled the ole case to use as a black jack during a couple bar fights when young but never needed to cut any one

 
Back to handguns . I carried at 3:00 in a iwb holster and was given some advise by the husband of dads office manager . He was a seasoned sergeant with our local sheriffs department when I started CC'n . He asked mye about what I carried and how along with other things but said he carried concealed with a IWB ay 3:00 when off duty . He was a big guy at about 6-4 and 275lb . He told me step forward with your strong side leg when holster so if you screw up you may only get a groove carved in your butt cheap or just a burn with a couple holes in your jeans ! That advise has stayed with me for 38 years .

Some time in the the 90's I was standing in the viewing gallery of a local range and a yahoo in a lane turned away from the safe direction of the lane 180 degrees towards me and started to clear his gun jam pointing it at me. At the time I was the resident range instructor (Not as an RO) and I went into the employee access door and shouted to the moron to put the gun down, he looked like a deer in the headlights and turned the muzzle at me again! I took a defensive position behind the armored range rear wall, put my hand on my Commander and shouted again for him to put it down. He did, I retrieved the gun, made it safe gave him his gun back without a slide on it and threw him out of the store.

After that the owner installed cameras in the range and he or an employee diligently monitored what was going on in there.

Anyway, back to the OP...

Six peeps watched the OP load a gun in public and said nothing? Egads. This stinks of a troll post because EVERY range I've been in since the turn of the century has video surveillance!

PS. Being a V-E-R-Y long time 1911 user I have a couple of M&P 2.0's in .40 with frame safeties and all OEM parts. I find the stock M&P to have more than satisfactory left ventricle / right ventricle accuracy with box stock parts at possible encounter distances.
 
Unfortunate situation while at work, I was inserting a fresh mag into my pistol when it fired and struck me in the leg. I inherited this gun from my grandfather who had a gunsmith replace the trigger to a APEX trigger. We’ve had several visits at the range no problems.
This shouldn’t have happened. Our office was full of people standing around and there really wasn’t a safe direction to point it other than towards myself.
What the ****! Would cause this to happen?
I was aware that there was a round in the chamber, but have inserted mags hundreds of times this way with no bad results like this.
I think anytime non stock parts are replaced it is a safety issue. I never replace stock parts on a carry gun other than sights.
I see the same thing on GlockTalk, something happens or something doesn’t work, 99% of the time it’s non OEM parts installed, whether it’s the part failure or the installation.
 
Just speculating here, but I suspect when the apex trigger was installed the striker/firing pin safety was removed in an effort to smooth the trigger pull. A very foolish thing to do in my opinion but it has been known that people sometimes do this. I would have the pistol disassembled by a qualified armor, inspected, and replace any missing parts.
Absolutely this. No way would I carry or use that firearm until it had been thoroughly stripped down and diagnosed.
 
The type of work I do, it’s dangerous.. I was getting ready to leave out in the field and the safest place to point was towards my led so it wasn’t anybody else’s.
Police report stated malfunction, as I had six witnesses and standing around me.
If you are standing, why not spread your legs a little and point the muzzle toward the floor between your feet?
