The face of the hammer isnt square to the bolt or the sides of the hammer so when the bolt slides back its riding on the edge of the hammer. After 2000 rounds or so, its starting to gouge into the back of the bolt.

Hardly any material needs to be removed to square it up, Im gonna take care of this when I polish everything up but I was wondering if anyone else has this problem?

Kinda disappointing.

Since this doesnt seem to be a common problem do you guys think this is a warranty issue? I can fix the hammer but the bolt will still be messed up.

I would contact S&W before I started tinkering with parts. Sounds like a warranty issue if it is gouging the bolt. If you mess around with it and they see that it has been polished and filed I imagine they will void your warranty. Perhaps you have some sort of alignment issue with your trigger group? I can't imagine how that could be out of alignment unless you have a malformed lower or something. Have you done any trigger mods?

I have around 3000 rounds through my gun, I looked at the bold at noticed a small amount of wear on the back bottom. I imagine this is normal from it cycling and re-cocking, but doesn't look major at all.

Some pictures would help for a diagnosis, it might be a non issue.
