please help me understand what happened

Great news on some awesome S&W customer service! Did they offer up what they found to have happened with the gun? Did they question, or mention reloads?

No mention or questions of anything just that it would be replaced with a new no-lock replacement (as mine was a no-lock) customer service A PLUS!!!
 
wow, glad to see s&w helped on that. But now I'd better go look, I am just getting to the end of a box of Alcan small pistol primers I have been using on 38 loads, 3gr on 146 wadcutters and a lot of 5.3gr on 125gr jacketed flat point. They all went "bang" no problem but the primers are of the same vintage as what you posted, but the cases are on their first and second reloads.
 
I don't think it was the ammo. I'm betting that the firing pin bushing was seated in the frame too deeply during manufacturing. Hence, the new gun.
