Revolver input required

I have the same 686+ four inch as the OP and also the 66-8 with the 4.25 inch barrel. If the OP only needs a six shooter, I would get the 66-8 snubbie with the 2.75 inch barrel. Easy to conceal and built like a tank. It has a full size forcing cone and can handle a steady diet of 357 Magnums if wanted or needed.
 
Would love to have a 3 inch 65 Ladysmith, or any 3 inch K frame. I just went back to carrying a 4 inch K frame. It’s not just about the revolver, but the entire set up. I wear flex waist jeans and have a Kore ratchet belt and a good holster. I’m considering getting a new 3 inch Colt Viper for concealed carry. The only drawback to the Colt, IMHO, is the cylinder release latch pulls back rather than pushes forward. The Viper will be spendy but perhaps easier to find than a 3 inch K/L frame.
 
Another vote for a 3" M65... one of the best carry revolvers ever made. No adjustable sights to snag on, and a full length ejector rod.

The PC Carry Comps are nice guns, but I dislike a comp on a carry gun. If you have to shoot from a compromised/entangled position, the comp port(s) may be pointed in an undesirable direction...
 
Cylinders for the 686 and 686+ are the same diameter, only bored with 6 or 7 holes respectively. The Plus is probably lighter when loaded.

A K-frame cylinder is about 3/16" smaller in diameter than the L-frame 686, with a more "svelte" profile. It's a little punchy to shoot with .357 loads.
