I think if it were me, I'd call them right away & tell them what happened and ALSO tell them you will send them pictures ! Also,if you were "badly hurt", I'd keep the medical records from your ER visit.

I had the same happen with my M & P 45. Remington ammo was to blame
OMG!!!!!!!!!!!!!!!!!!!!!!!!!!! My CC got charged $359.00 July 3rd by S&W!!!!!!! I wonder what that means.....
Check the barrel
That you "couldn't hit the broad side of a barn" tells me that perhaps the barrel was thoroughly fouled after shooting lead bullets, perhaps at a previous range session? Shooting jacketed .40s through a lead-foulded bore is a sure recipe for a kaboom. This gun either fired out of battery (unlikely) or pre-maturely opened before the gasses dissipated, indicative of blockage of some sort.

Double Charge???
The primer doesn't look flattened-wouldn't a double charge severely flatten the primer. Looks as though a "slam fire" may have occurred (unsupported case head). I've heard somewhere that this type of ka-boom has happened previously in the Shield 40's. Not with great regularity, but has happened.

I can't believe it.... but this may finally come to a somewhat happy conclusion...!! I would expect 2001 should be getting a call any day now... maybe the powers to be can put a bow on this thread and close it when we know for sure...

Picked up the 2nd 638 from our mutual friend today. Mine took what 5-6 weeks? this 1 was here in less than a week!!!

Well guys I got the call Monday that my Shield was at my dealer. I been working a lot and haven't had a chance to post. I will pick it up today. Thanks again to everyone who took part in this. Thanks Jack again for everything you have done. I finally get a gun back. 10-2-13 to 7-7-14 and its finally here.
