All suggestions and advise are appreciated. I know it is impossible to diagnosis problems over the internet. Also, I just enough about the internal working of this gun to get myself in trouble. i have decided to send the pistol tl Clark Custom Guns to get repaired.
Thanks again.

Still Have 41 Problems

Are you absolutely sure you aren't accidentally engaging, or even touching the safety lever at all while you are shooting it? I have found on mine it is quite easy to slightly move the safety lever if you aren't careful and it results in a similar condition as yours. The hammer will fall, but the round doesn't go off. I have learned to avoid touching the safety lever at all.

I had a similar situation occur when I installed custom grips on my 41. The grips were cut so my thumb engaged the safety after firing a shot. This was especially irking when shooting the Timed Fire and Rapid Fire portions of Bullseye matches at my local gun club. I solved the problem by disabling the safety by placing a folded piece of paper under the safety lever and tightening the screw.
