I'm a little more inclined to be curious as to why the OPs physician asked the question. Some posters are either apologising for the invasion of privacy regarding gun ownership, advocating lying (which some crazy people have an aversion to) and/or being apathetic and facilitating a crime in the state of Florida.
All free advice, and worth the price....

but......why, and who initiated this "form"?
 
coming up on 65 yrs old. OMG I really am old.....anyway I have never had a civilian doc or any one of my VA docs ask if I was a firearms owner...never.
 
I was asked once by a doc ( a good guy ) who is an avid hunter and who had discussed my pistol competition with in the past.

I asked him if docs had gun safety as part of the medical school curriculum.

(Yeah, it is a lie but it is none of their business and you are not under oath. You aren't going to be arrested, sued, fired or anything else for what you say. People often exaggerate, under report or misrepresent what they tell their doctors, dentists, lawyers and accountants. Or, you added the guns after you answered the question, or they weren't in the house on the day you were asked, or...)

Maybe the doctor is violating state law by asking, maybe not. If your goal is to enforce the law then make a thing out of it and follow through with some sort of complaint or lawsuit...I support you 100%. If your goal is to give the doctor an answer he can put on some form because he thinks he has to, then tell him whatever you want and move on...I support you 100%.

If you are uneasy being untruthful with your doctor, then ask him who is asking the question, him or whoever wrote the form. If he says it is whoever wrote the form, say "Then the answer is no." He will get the message.

I am waiting for my doctor to ask me that question.
If and when he does, I will lean forward, look him in the eye,
and ask him if he has any kiddie porn in his house.
That should end the conversation very quickly. :D

Well now that I know what to expect i printed me out a copy
of this texas map where all the texas gun owners are represented by a little red dot.

And I will just keep that little map in my billfold and present it to the doctor when they ask about guns, leaving the map with
him to send back to whoever in the federal government was
requesting the information.
 
I'm on Medicare and so is my wife we both got our yearly physicals a month ago . No questions about guns!
