I was looking at this gun and found something I haven't seen before. Right under and to the rear of the thumb piece there is a round plug in the side plate. I have only seen locks above the thumb piece. Any ideas, possibly a custom shop item? I'll try to get a pic if they have not sold it.

Could you be seeing one of the frame pins? In this photo, this UC gun has some slight color contrast between them and the frame and you can see a small gray circle just under the thumbpiece, along with two under the cylinder window.

Could be, a hammer pin was broken and it was drilled through and replaced with a through pin. There used to be a hammer pin boss integral to the left side of the frame inside….sometimes these hammer pins are bent or broken, usually from some knuckle-head forcing the side plate on incorrectly. The softer alloys are drilled and the boss with hammer pin is pressed in from the right-I believe this to be the method they use-and this may be the similar example of what you see….maybe a repaired 686, or maybe they do new L frames like that nowadays…cheers

The Model 686 Plus was introduced in 1996, which pre-dates the internal lock (2001/2002), so there were quite a few manufactured without the lock. Here's a modern 686 Plus with the internal lock (note the location), and I do not see a frame pin below and behind the cylinder release. However, the OP's pin/plug is a mystery...perhaps a frame pin, perhaps a repair?

The " below the cylinder" release threw me as well but in my head thought if it was a broken hammer stud SW would have probably replaced the frame under warranty,
Until we see pics or the op can explain that it is above or below the cylinder release its a mystery.
