I've just started concealed carry of a new SW Model 638-3 JFrame. I know reloads in actual self defense shootings are rare, but what do most of you folks use to carry extra ammo (if you do) and where do you carry it?

I use a Bianchi Speed Strip carried in my right front pocket. The strip holds six rounds and I carry it with all six even though the cylinder only holds five.
It's a personal preference decision . . . use what works best for you.

A speed strip rides well in the oversized watch/cell phone pockets that are found in most jeans these days.

I New York Carry (2) "J" frames... One in my strong side front pocket in a Nemisis Holster and the 2nd J at 3:30 belt position in a Bianchi... I carry 2 Bianchi Speed Strips stoked with 158gr standard pressure Gold Dots because I find that they reload much faster in the cylinder holes compared to soft cast hollow points or full wads.
Primary is loaded with 150gr Buffalo Bore hot hard cast full wad cutters with the 2nd "J" loaded with Buffalo Bore +P 158 LSWCHP's...

I too use a Speed strip but I also use a 2x2x2 belt pouch. I wear it daily on the right front, near the belt buckle, no one has ever asked about it.

I pocket carry a j frame, a Desantis speed loader pouch is on the belt at the 11:00 position...off side pocket sometimes a speed strip also. The speed loader is a Safariland comp1 and the pouch # A35..even a tee shirt hides the pouch.

I also pocket carry a hump back. I carry two 6 shot speed strips in my pockets. I also have speed loaders and extra .38 ammo in my vehicles. I like having plenty of ammo around. I guess I get this from my father who was career 82nd Airborne. He was really big on having spare ammo around especially when out hunting.

Where you wear your reloads is going to depend on HOW you do your reloads. So what I do might be different than you.
But since you asked. Currently I wear my 637 AIWB and I have 2 speedloaders in my left front pocket. I use a weak hand reload as of lately.
For a long time I used a universal reload and kept my reloads in my front right pocket. 2 Bianchi speed strips. At times I did 4 rounds in each and other times I did 5. 1 in the coin pocket and 1 in the pocket.
There was also a time I wore my speedloaders in my front right pocket. I'm not exactly sure why I went to speed strips for a while. I think it was because my bedside gun is a model 19 and if I actually had my pants on while grabbing it for self defense I could use the speed strips in either gun.
I have a speed strip stashed in my living room for that purpose. It won't matter if I have my J frame or Model 19 and need to reload.

I'm the same way... You can never have too much ammo or too much water!
I just got my first "Humpback" yesterday from a sale I heard about @ Adventure Outdoors in Smyrna GA... I like it so darn much I should have bought two of these M642's. It came timed to perfection, the trigger break is on par with my 77' and 78' manufactured M36's... lock up is darn close to the aforementioned 70's J's which is I never thought I'd get from a "current" production Smith. Best yet, no "He(( Hole" in the frame for me to have to snatch out and plug up with one of J.D.'s Original Precision lock deletes (of which is a must do for me with any "Hole" equipped Smith's)... Carrying (2) "J" frames in the burbs... I carry my "N" frame M629 snubbie and one of my "J" back ups for the "Woods" has become my Modus Operandi. My semi-auto pistols have been relegated to Home Defense guns staged platforms since my Smith/Revo Revelation!
I tend to carry around 30 rounds for the burbs and 50+ in the backpacks/cargo pants for the woods at a minimum.

My Airweight usually is the extra ammo.
OP is correct about reloads of revolvers in actual situations being rare. Indeed, they're almost impossible, or more precisely, almost impossible to survive.
Take that j frame to a gun show, swap it for a Shield, or some other small 9mm.

I use a left-handed reload method, specifically the deBethencourt method, so I carry my reloads behind my left hip (it's consistent with how I carry reloads for my semi-auto...I like consistency). When I carry my 642, I carry reloads either on a Tuff Products Quickstrip in a Simply Rugged belt pouch or a pair of Safariland Comp I speedloaders in JOX kydex pouches. Ammo is Speer 135gr SB-GDHP +P.
An excellent point. I use a strong hand reload so my speed strip is in my right pocket.
Whatever you decide works best for you, practice and be consistent. If you should ever need it in a hurry, you don't want to wonder where you put it today and waste valuable time hunting for it.

When I carry my 640 or 637, I usually carry a HKS speedloader and a Bianchi speedstrip. Usually I carry them on my right side in a pocket. All are primed with Speer GD SB 135 grain ammo.
If I have a cover garment I'll carry a 5 round HKS speedloader in a Wild Bill's Concealment slip over carrier.
This one is for a 6 round K frame speed loader (with a Model 66), but the concept is the same. It rides with 2 or 3 rounds inside the belt so it's low profile.
Below is the smaller 5 round version. If I do not have a cover garment, I tuck my short over the revolver and carry a speed strip in a pocket. These are the only 5 round speed strips I own. I'm more inclined to use a 6 round speed strip and leave the end hole empty to provide for more/easier grip on the strip.
