From the pictures and the initial couple posts it was a complete mess up. The OP fired the gun, cranked the hammer back and then became aware something was wrong. He is not really clear on what his clue was (I suspect it would not go to full cock). He then tried to swing the cylinder out. When it would not come out he beat on it with a mallet. When that did not work he wanted you all to tell him how to fix it. he chose the advice that looked easiest, running in a big lag screw, into the squib and surprise, he did not do that quite right, and pulled the lag screw loose.
