Prayers needed.

I'm saying prayers for your sister-in-law and also for your nephew. A decision like that is difficult for anyone. It will be tremendously difficult for a teenager. I had to make that choice with my Mom, and it was (and still is to some degree) painful. I was 45. I can't imagine the stress of having to make that call at 18.

Those of us who have not done so should really consider a "living will" or medical directive or whatever they call it these days to spare our families from having to make that choice.

My nephew has made his decision and he want's to make her as comfortable as possible and let the good lord do his work and if God calls her home he will let her go. He knows the pain and struggles she has dealt with on this earth and he feels she may be ready to go home but he also knows how much she loves him, his brother and her family.
