1. Check ejector rod (tightening): which you did
2. Check Star (both around and under it) for crud build up
3. Check for bent ejector rod
4. Check for forward/back excessive cylinder movement
5. Check front of cylinder and barrel for any excessive lead build up.
6. Spray out internals with some solvent/oil
7. Last desperate attempt would be to push both the hammer and rebound slide closer to the frame.
*You could remove the side plate and post some pics or do a video and post then give us the link and we could maybe help out some more
After these, I'm afraid that, its something mechanical (spring, hammer bind, hand bind or something else mechanical that would involve a disassembly and inspection.
